Our expert pulmonary rehab team includes physicians, dieticians, respiratory therapists, pharmacists, and social workers. This multidisciplinary program offers pulmonary self-management skills and education, monitored exercise sessions and Better Breathers Society support group meetings.
Our friendly credentialed staff provides participants with the necessary information and tools to achieve better physical and emotional health.
Physicians referring patients to Covenant HealthCare’s Pulmonary Rehabilitation program can expect the following:
- Patient Assessment with AACVPR Recommended Tools
- Individualized Treatment Plans
- Patient Education Including Self-Management Tools
- Communication of Patient Assessment Scoring and Progress
- Patient exercise monitoring by ACLS certified staff
A physician’s referral and a pulmonary function test will be required prior to enrollment in the Pulmonary Rehabilitation program.
